Arachnoiditis

Arachnoiditis is a painful condition characterized by inflammation of the arachnoid membrane, which is one of the three layers of tissue (meninges) that surround and protect the brain and spinal cord. This inflammation can lead to severe, chronic pain and various neurological symptoms due to irritation of the nerves in the spinal canal.

Arachnoiditis is often encountered in individuals who have had severe compromise of their spinal nerves in the past. Many of these individuals have had decompressive surgery, but the nerve roots become inflamed and ”stick” together abnormally. This is challenging to treat.

Symptoms:

Chronic Pain: Persistent pain in the lower back, legs, or buttocks; pain may be sharp, burning, or aching.
Neurological Symptoms:
  • Numbness and tingling sensations in the legs or feet.
  • Weakness in the lower extremities.
  • Sensory changes, such as altered perception of temperature and pain.

Other Symptoms:

Bowel and Bladder Dysfunction: Difficulty controlling bowel or bladder functions, including urinary incontinence or constipation.
Saddle Anesthesia: Loss of sensation in the areas that would come into contact with a saddle (inner thighs, perineum).
Difficulty Walking: Impaired mobility due to pain, weakness, or balance issues.
Other Symptoms: Fatigue, sleep disturbances, and mood changes, such as anxiety or depression.

Causes:

  • Infections: Infections such as meningitis can cause inflammation of the arachnoid membrane.
  • Spinal Procedures: Surgical interventions or procedures like lumbar punctures (spinal taps) or epidural injections can lead to arachnoiditis.
  • Trauma: Spinal cord injuries or trauma to the spine can trigger inflammatory responses in the arachnoid membrane.
  • Chemical Irritants: Exposure to irritants, such as contrast dyes used in imaging studies, can cause inflammation.
  • Herniation of Discs: Prolapsed or herniated discs in the spine can lead to inflammation and irritation of the arachnoid membrane.

Treatments:

There are several effective ways to alleviate the pain, including a variety of treatments beyond just medication. Treatment options for Arachnoiditis includes:
